Application for prospective members of Guan Ho Ha. Please ONLY fill out this application if you have been on the waitlist and been contact by the Membership Secretary. Upon completion of this application you will be put into pending status. Your application will only move forward once it has been confirmed that you were on the waitlist and that you are up for membership.

Payment of $255 is due prior to the meeting when you are voted in. You will not be moved to full active membership until payment is received.
